If you are a member then you may select one of the following immunizations for an analysis as to whether or not you qualify for recieving treatment. The treatment will be ordered to your local home care agency, pharmacy, or urgent care clinic. (You will not have to have a conference with the physician if you meet the criteria for immunization). However, you will have to have submitted a personal medical history to the Cyberspace Telemedical Office before any orders can be provided. The cost per immunization ordered is $10.00 and does not include the cost of the vaccine or procedure. You will have to pay these additional costs directly to the provider of the vaccine.
