opinion medical consultations. Physician specialists provide
a written consultation report based upon data entered into the
CyberDoc Health Problem Questionaire and the Comprehensive Medical
History Form. Important Disclaimer--The results of the second
medical opinion are meant to enhance the quality of your healthcare
and should be discussed with your primary health care providers.
The results are not a substitute for seeking medical care from
appropriate health professionals.
Members are entitled to one free consultation per year
and will be charged $40 for each subsequent consultation. Members'
Comprehensive Medical Record information is maintained in the
Cyberspace Telemedical Office and it is not necessary to re-enter
this information. However, members are encouraged to periodically update their Health
Records to optimize the relevance of your Personalized HealthCare
Services.
